Your First Visit to Our Darien Practice
New patients will come into our office and be greeted by our staff. They will need to produce a form of identification such as a driver’s license and any insurance card(s) that they want to consider for assignment.
We will ask for them to fill out application forms which include demographics, health history, informed terms of acceptance of care, acknowledgement of office policy, and the mandatory HIPAA receipt page.
The patient should bring in pertinent medical diagnostic information such as previous X-rays, MRIs, CAT Scans, EMG/NCVs, blood test results, etc. to assist the doctor to make an accurate determination. The information is processed and the patient meets with the doctor.
The doctor determines if the patient is qualified to be given care, and if qualified, the doctor proceeds with the necessary physical and diagnostic examinations. Treatment may or may not begin on this day depending on the complexity of the condition to be treated given the information. The patient schedules for a second visit at the front desk to allow the doctor time to assess the information given.
We will do our best to expeditiously research your insurance benefits as a courtesy.
Patients may also pre-fax their insurance card at 200% magnification, both sides, to pre-determine their coverage. Depending on the carrier and the specific plan, a co-pay, deductible or good-faith payment is expected as mandated by your insurance company. Services that are paid for at the time of service may be entitled to a time of service discount. We also have discount plans to some of our services available. Please inquire at the front desk for details.
The total time spent is typically 25 to 35 minutes.
